Assisted Living At-Home

This is our specialized around the clock live-in care program designed for busy professional families who do not want the stress of family caregiving but want
to see their loved ones thrive outside of the nursing home while they enjoy work life balance and maintain their role as family members. Care is delivered in 24 hour shifts

Aging-in place services

 This is our customized hourly non-medical care services designed to help older adults to age in place and stay independent in the place they know by heart, their home. You pick your hourly rate, the caregiver and the rates that fits their budget and situation.

Post Hospital Post Rehab Care program

Hospitals are turning into a giant ICU so patients who are admitted are much sicker today than ten or fifteen years ago.
The problem: For every day that an older adult is in a hospital, he/she loses 3 – 5 (possibly more for people with movement disorders) days of their pre-hospital level of body functioning. After only a few days in the hospital, your loved one may not be able to walk or to take care of his/her personal care needs. In addition, some may begin to show symptoms of dementia with or after a hospital stay. 

Statistics have shown that 85% of the people who go into the rehab facility never return to their homes . . . . they get transferred into a long-term care facility because they need more care than their family caregivers can provide. 

What if you are not prepared for that or your loved one does not want to move into a facility but needs a lot of care after a hospital stay?

The SOLUTION: We created our post rehab and post hospital care program .
This service is designed by a nurse practitioner and the care is performed by our Life Enhancement Caregivers. This professionally coordinated care allows the senior to recover in the comfort of their home and offers their adult caregiver the peace of mind they deserve. We work alongside Medicare skilled services to help older adults manage their chronic illnesses, participate in their home wellness program, stay compliant with medications, stay active, stay well, and stay out of the emergency room.
