Isoken Ogbomo, founder and family nurse practitioner

Why we began

Families deserved a better way to navigate complex care.

In-Home Caregivers of Illinois was founded by Isoken Ogbomo, MSN, FNP-C, a family nurse practitioner, registered nurse, geriatric care consultant, and long-distance family caregiver.

Throughout her career, Isoken saw the stress families faced when a loved one came home from the hospital, began showing signs of memory loss, or developed a complex health condition. Families were often left to manage many moving parts without one trusted person helping them see the whole picture.

She created this company to bring clinical knowledge, compassionate home support, and clear guidance together. The goal was never simply to provide hours of care. The goal was to help each client follow a safer, more dignified path to a better quality of life.

In Service and In Care,
Isoken Ogbomo

Our approach begins with listening.

Every person has a different health history, home life, personality, and vision for aging. One standard care plan cannot serve everyone well.

We learn about the client’s health, daily routine, strengths, concerns, preferences, and goals. We then create a personal care pathway that may include home care, companionship, wellness support, disease-specific care, nursing services, or help after a hospital stay.

Whole-person support

  • Physical wellness, including mobility, nutrition, hydration, sleep, comfort, and personal care
  • Health awareness through observation of symptoms, function, mood, and mental status
  • Emotional wellness through companionship, reassurance, and meaningful connection
  • Daily independence that helps clients remain involved in the activities they value
  • Family confidence through clear communication and regular updates
  • Coordination with approved healthcare and community partners when needed

Our integrated geriatric care pathway

Seven connected steps. One personal plan.

Our pathway brings order and accountability to care. It helps the client, family, Care Partners, nurses, and approved healthcare providers work toward the same goals.

01 Referral & initial conversation
02 Whole-person assessment
03 Meaningful wellness goals
04 Personal care pathway
05 Coordinated care delivery
06 Reassessment & outcomes
07 Reporting & collaboration

I can’t adequately express how grateful I am for the help we received for my mother from Hattie, Leslie, and Isoken. I think the work that they do could more accurately be described as a calling, and I will be grateful to these three women in particular and this company in general for the rest of my life.

The agency provides companionship for my mom. The staff member is very pleasant, always on time and very courteous. The caregiver has become a reliable source for my mom’s care. The agency is very responsive to my needs and always very accommodating.

I use In-Home Caregivers to assist with care for my dad. All of the caregivers and Isoken have been professional and kind to him. Communication is especially important since I live out of state and they have been very responsive via email and phone. Finding quality care for aging parents is incredibly difficult. I feel fortunate we were referred to In-Home Caregivers.

Isoken is not only professional and organised, but genuinely caring and accommodating. Hattie, too, is a pleasure to speak with and is quick to think of solutions for the various issues which crop up when you have general difficulties looking after a loved one. The carers they sent each day to look after my mother were professional and kind—there were so many who had a personal, affectionate way about them that I couldn’t possibly list all their names. Having Isoken and Hattie in my life during such a difficult period felt like being watched over by my favourite aunties—the ones who know how to get stuff done and make you feel better. I cannot recommend this company enough.

I would like to personally commend the work that In-Home Caregivers of Illinois do. On a particular day when there was ice in my walkway, I called them to cancel care for the safety of the caregiver. Instead they went to purchase salt and sent someone to remove the ice from my entrance so I could get care that day. They went above and beyond the call of duty to make sure I didn’t go without care. Whenever they send a new caregiver, they always come with a supervisor to train the new caregiver so I don’t have to do that. They are a very professional and dependable caregiving establishment. They care more about the person than the monetary compensation.

This is my first caregiving experience and I had no idea what to expect. I’m very pleased with the care and communication. Great choice for caregiving.

I just want to recommend this agency to help provide in-home services for your loved ones. Hattie and her team members were so professional. They took the time to answer all my questions and although we worked with them for a short-term period, this was the best agency I ever worked with. I have numerous experiences with different home-health agencies, and In-Home helped me realize that not all agencies are the same. With In-Home, you can count on them to show up for your family members’ needs and they are excellent with communication.

My experience working with In-Home Caregivers has been amazing. My great-aunt is dealing with dementia and is profoundly deaf. Hattie and Isoken are incredibly flexible and so patient and understanding—I cannot emphasize that enough. My aunt can be incredibly difficult in certain regards, and the empathy and grace with which these women handle and serve her is truly something else. Lataya and Maribel have also worked with her and were very receptive to meeting and exceeding her needs. I strongly recommend trusting these women into the homes of your loved ones. They are such a godsend.

In-Home Caregivers of Illinois were excellent to work with when trying to find in-home care for Mom. Isoken and her team listened to what we needed and were able to find us the help that was best for Mom. Bronda was wonderful with her care and companionship. I would highly recommend Isoken and her team.

I highly recommend In-Home Caregivers of Illinois. The level of care, compassion, flexibility and professionalism provided was very much appreciated. I searched for assistance to help me care for my mom who lives with dementia. It was difficult for me to trust her care to someone else. Isoken took the time to understand my mom’s needs as well as mine. She showed genuine interest in my mom’s care and compassion for her delicate condition. The caregiver selected, Helen, has been nothing but amazing. Helen is patient with my mom and I know I don’t have to worry when Helen is with her. Isoken has continued to check in to make sure my mom is happy. Helen and Isoken made me trust the process and feel comfortable with others helping me with my mom.

Questions families ask

Clear answers before you call.

Who founded In-Home Caregivers of Illinois?

Isoken Ogbomo, MSN, FNP-C, a family nurse practitioner, registered nurse, geriatric care consultant, and long-distance family caregiver. The company is the DBA of Complete Health Service Inc.

Are you a shift-fill staffing agency?

No. We are a nurse-practitioner-led home care and home nursing company. Care Partners are matched on skill and personality, a supervisor introduces each new Care Partner in the home, and the team stays involved after care begins.

What does nurse-practitioner-led mean?

Clinical insight shapes the plan: assessment, matching, and follow-up are not separated from the hours of care. The founder is a family nurse practitioner. That does not replace the client’s own physician.

Where do you serve?

Cook, Will, DuPage, Grundy, Kendall, Kane, and LaSalle counties, from offices in Bolingbrook and Morris, Illinois.

How do we start?

Call 630-300-0446 or send a care inquiry. You do not need a diagnosis, a referral, or a set number of hours. Tell us what has changed and what a better day would look like.
